How do I get the companion airline ticket?
A registration form will be mailed to you after meeting the 60-day account
requirements. Simply complete and mail in the registration form. You will then
receive an Activated Voucher in the mail within 14 days of registering. You can
go online or call a toll-free number listed on the Activated Voucher to purchase
one roundtrip, coach-class ticket to receive your complimentary companion
ticket for the same flight.

Whom do I contact if I don’t receive my registration form or voucher?
Please contact your Union Bank banker.

Can I call the airline directly to make airline reservations?
No. All travel must be booked through the 800# or through the online
booking site, which will be listed on the voucher.

Is the companion airline registration form transferable?
Yes, you are able to transfer/give the registration form to a friend
or family member. Simply provide him or her with your registration form.
Once airline travel is booked and a ticket is issued, it cannot be transferred.

How long do I have to use the companion airline ticket?
Once you register, you will have one year to complete your travel.

Are there any restrictions when I can travel?
Seasonal and holiday blackouts may apply.

Do I need to use my companion airline ticket and hotel stay at the same time?
No. The two offers are independent of each other and may be used
simultaneously or separately.

Are there any fees I’m required to pay?
Yes. The companion fare tickets apply to the base fare only. A $9.95 per
ticket processing fee plus taxes and fees charged by the airlines will apply
to both the companion ticket and the first ticket.

Will I receive a 1099-INT for the value of this offer?
Yes. Registration will trigger the receipt of a 1099-INT with a value of $257.
You will be issued a 1099-INT if you register for either the companion airline
ticket, the 2 night hotel stay or both. The value will be $257 for any registration.
2 Night Hotel Stay: Frequently Asked Questions

How do I book the 2 night hotel stay?
A registration form will be mailed to you after meeting the 60-day account
requirements. Simply complete and mail in the registration form. Once you
have registered, you will receive a packet of information that includes the
list of hotels and resorts from which you can choose. When you select your
hotel destination and the dates you want to go, just complete the Vacation
Request form, which will be included in the packet, and mail or fax it at
least 28 days in advance of your earliest requested arrival date. See packet
instructions for details.

What types of hotels can I choose from?
You will be able to select from resorts and hotels managed by such reliable
names as Wyndham, Marriott, Sheraton, Radisson, Choice and Holiday Inn as
well as some independent hotels.

Does the registration form expire?
Yes, the expiration date is printed on the form. You must register
by the expiration date.

Do I have to spend 2 consecutive nights at the same hotel?
Yes.

Is the 2 night hotel stay transferable?
Yes, you are able to transfer/give the 2 night hotel registration
form to a friend or family member. Simply provide him or her
with your registration form. However, once your hotel request
has been made, it is not transferable.
